What Do I Need To Enroll My Child In School?

Please make sure you have these documents ready when it's time to enroll your child in Success Preparatory Academy.
  • Birth certificate
  • Social Security Card (if available*)
  • Parent/guardian ID (something with name and photo, does not need to be state-issued)
  • Immunization (shot) records
  • Individual Education Plan (IEP) for special needs students
  • Proof of local address in Orleans Parish (such as a electricity bill, telephone bill, copy of your lease, etc.)
  • Name, address, and contact information for your child's last school
  • Most recent report card
  • Latest test results (LEAP, IOWA, other state tests)
*Social Security Number is NOT required for enrollment but is used for student identification purposes if provided. (U.S. Code § 552a)
